Transfer Application Process

Deadlines

Application deadlines for 2014 enrollment

The Application and all required application documents and materials (see below) are due by the deadline date in order to begin the application review process for professional programs (Pharmacy, Physical Therapy, Occupational Therapy and Physician Assistant.)

Application

There are several options for submitting your transfer application to USciences. Please review carefully.

  • Completed University online or paper application for admission (If you submit a paper application, please include a $45 nonrefundable application fee with your application (make your check or money order payable to USciences and include your full name in the memo section). The fee is waived for online applications.
  • Students applying to the 3rd year of the doctor of pharmacy program must apply via the Pharmacy College Application Service [PharmCAS]  (Note: PharmD transfer applicants must pay fees required through the PharmCAS)
  • Students applying to the graduate phase of the Physician Assistant program must apply via the Central Application Service for Physician Assistant [CASPA]  (Note: Physician Assistant graduate applicants must pay fees required through the CASPA). A supplemental application must also be completed.
  • Application Reactivation Form: If you wish to reactivate your application because you were not admitted, were wait-listed, or decided not to enroll, you may use this form to reactivate your application once, without an additional application fee, for the next academic year. Note: Applicants to the professional phase of the PharmD and Graduate Physician Assistant Studies program may not use this form. These applicants should re-apply via the Pharmacy College Application Service (PharmCAS) and CASPA respectively.

Required Documents

In addition to your application, you need to submit the following for your transfer application to be considered complete:
Check Official transcripts from all colleges and/or universities attended
Check

An official high school transcript or an official copy of a General Equivalency Diploma (GED) (if you will not have an Associate's or Bachelor's degree prior to your enrollment at USciences.)

Check Official SAT or ACT scores (if you will not have an Associate's or Bachelor's degree prior to your enrollment at USciences.) Please contact the College Board at 866.756.7346 or ACT Assessment at 319.337.1313 to obtain your scores).

Check All items listed above.
Check One to three letters of recommendation. Recommendations may be submitted using our recommendation form.

Completed Pre-Professional Form:

  • Occupational Therapy Pre-Professional Form
  • Occupational Therapy Post-Baccalaureate Pre-Professional Form
  • Physician Assistant Pre-Professional Form (for BSHS/MSPAS program only)
  • Physical Therapy Pre-Professional Form
  • Physicial Therapy Post-Baccalaureate Pre-Professional Form
  • Check A professional goal statement—The statement should explain in detail your interest in the professional program to which you are applying, how you developed this interest, your career goals, and any attributes and accomplishments you believe will enhance your future academic and career success. Also please tell us why you have chosen to apply to USciences.
    Check A resume—Students that complete the volunteer and work experience portion of the application do not need to submit a separate resume.
