Transfer Application Process


Deadlines

    Doctor of Physical Therapy (DPT) May, 2017 November 1, 2016*
    Master/Doctor of Occupational Therapy (MOT/DrOT) May/June 2017 January 1, 2017*
    Doctor of Pharmacy (PharmD) August, 2017 March 1, 2017*
    Physician Assistant (MSPAS-post-baccalaureate program) August, 2017 November 1, 2016
    Undergraduate Programs (Spring 2017) January, 2017 November 1, 2016

    *The first professional year (P1) of the PharmD and OT programs is year 3, the P1 year for the DPT program is year 4.

    The Application and all required application documents and materials (see below) are due by the deadline date in order to begin the application review process for professional programs (Pharmacy, Physical Therapy, Occupational Therapy and Physician Assistant.)

Application

There are several options for submitting your transfer application to USciences. Please review carefully.

  • Completed University online or paper application for admission (If you submit a paper application, please include a $45 nonrefundable application fee with your application. Please make your check or money order payable to USciences and include your full name in the memo section. The fee is waived for online applications.
  • Students applying to the 3rd year of the Doctor of Pharmacy program must apply via the Pharmacy College Application Service [PharmCAS]  Note: PharmD transfer applicants must pay fees required by the PharmCAS. No supplemental application is required.
  • Students applying to the 3rd year of the Master’s or Doctorate of Occupational Therapy program must apply via the Occupational Therapy Centralized Application Service [OTCAS]. Note: OT transfer applicants must pay fees required by the OTCAS. No supplemental application s required. 
  • Students applying to the 4th year of the Doctor of Physical Therapy program must apply via the Physical Therapy Centralized Application Service [PTCAS]. Note: PT transfer applicants must pay fees required by the PTCAS. No supplemental application is required. 
  • Students applying to the graduate phase of the Physician Assistant program must apply via the Central Application Service for Physician Assistant [CASPA]  Note: Physician Assistant graduate applicants must pay fees required through the CASPA. A supplemental application will be requested of qualified candidates.
  • Applicants interested in taking courses as a non-matriculated student should apply using the Online Application.
  • Applicants interested in the Healthcare Navigation Certificate should apply using the Online Application.

Required Documents

In addition to your application, you need to submit the following for your transfer application to be considered complete:
Check Official transcripts from all colleges and/or universities attended.
Check

An official high school transcript or an official copy of a General Equivalency Diploma (GED) (if you have an Associate’s degree or 60+ college credits prior to your enrollment at USciences these transcripts are waived)

Check Official SAT or ACT scores (if you have 24 or more credits you do not have to submit scores.) Please contact the College Board at 866.756.7346 or ACT Assessment at 319.337.1313 to obtain your scores).

Transferring to the undergraduate level of a Professional Program (Doctor of Pharmacy, Doctor of Physical Therapy, Doctor or Master's of Occupational Therapy, Physician Assistant Studies)

Check One to three letters of recommendation. Recommendations may be submitted using our recommendation form.

A professional goal statement—The statement should explain in detail your interest in the professional program to which you are applying, how you developed this interest, your career goals, and any attributes and accomplishments you believe will enhance your future academic and career success. Also please tell us why you have chosen to apply to USciences.

*Students applying via a CAS service should follow the instructions on CAS and send all documents directly to that service.
