Applying to a Bachelor’s Degree Program—Apply by June 1st for priority Review

Here is what you need to submit for your transfer application to be considered complete:

	Completed online or paper application for admission (If you submit a paper application, please include a $45 nonrefundable application fee with your application (make your check or money order payable to USciences and include your full name in the memo section). The fee is waived for online applications.
	Official transcripts from all colleges and/or universities attended
	An official high school transcript or an official copy of a General Equivalency Diploma (GED) (If you have not completed an associate’s degree.)
	Official SAT or ACT scores (If you have not completed an associate’s degree.) Please contact the College Board at 866.756.7346 or ACT Assessment at 319.337.1313 to obtain your scores).

Applying to a Professional Program (Doctor of Pharmacy, Doctor of Physical Therapy, Doctor of Occupational Therapy, Physician Assistant Studies)

	Students applying to the 3rd year of the doctor of pharmacy program must apply via the Pharmacy College Application Service [PharmCAS]  (Note: PharmD transfer applicants must pay fees required through the PharmCAS
	One to three letters of recommendation. Recommendations may be submitted using our recommendation form.
	Personal statement
	Completed Pre-Professional Form: OT Pre-Professional Form OT Post-Baccalaureate Pre-Professional Form PA Pre-Professional Form PT Pre-Professional Form PT Post-Baccalaureate Pre-Professional Form
	A professional goal statement—The statement should explain in detail your interest in the professional program to which you are applying, how you developed this interest, your career goals, and any attributes and accomplishments you believe will enhance your future academic and career success. Also please tell us why you have chosen to apply to USciences.  (This replaces the personal statement listed above.)
	A resume—Students that complete the volunteer and work experience portion of the application do not need to submit a separate resume.

Application Deadlines

Most of our programs have a rolling admission policy, which means that your transfer application will be reviewed when all of the necessary components are received, and you will be notified quickly of our decision. Only our professional programs have application deadlines:
