Health and Immunization Requirements for Entering
Students
University of the Sciences requires all entering full-time
undergraduate and graduate students, including all international
students, to provide a complete medical history and health
evaluation (i.e. Physical Exam) and to fulfill the following
immunization requirements:
- Tuberculosis screening: For students
with signs or symptoms of active tuberculosis, students
from high-risk groups as defined by the Centers for
Disease Control, or students entering the health professions
a tuberculin skin test required. A
chest x-ray is required if tuberculin skin test is

- Measles Mumps Rubella vaccine: Two
doses of MMR vaccine or individual vaccines
- Tetanus-Diptheria-Pertussis: Primary
series of four doses and Tdap booster required within
the last five years
- Meningitis: Vaccine or signed waiver
of declination is required for all residential
students
- Hepatitis B vaccine: Three doses
or a positive blood test
- Polio vaccine: Three doses (primary
series) or three primary
- Varicella: Healthcare professional
majors must provide documentation of two doses of varicella
vaccine or positive titer proving immunity. All
other students must provide documentation proving history
of disease, two doses of varicella vaccine, or a positive
titer.
The pre-matriculation immunization requirement is based
on the recommendations of the Centers for Disease Control
(CDC) and Commonwealth of Pennsylvania Department of
Health, Division of Immunizations and the American College
Health Association. You can satisfy this requirement
by completing the Medical
History Questionnaire form,
which includes the immunization requirements.
Students who do not submit a fully completed health
form by August 10, 2011 will be charged a late fee of
$25.00 and registration will be withheld until all health
requirements are met (completed health form and proof
of all required vaccinations).
