Advisor of the Year Nomination Form

Purpose: To recognize and honor a professional or faculty advisor of USciences for excellence in academic advising. The nominee can be your assigned academic advisor or another advisor with whom you interact in an informal way for academic advising issues.

Procedure: Students, staff, and faculty may complete nominations. Advisors may nominate themselves. The Academic Advising Committee will review the nominations and the winner will be announced in June 2013.

Completed forms can be submitted on-line by April 30, 2013.
