Advanced Pharmacy Practice Experience (APPE)

The APPE program is for 4th professional year Pharm D candidates and consists of seven 5 week rotations. Within these seven rotations, there are four core required rotations that are to be completed within the current Philadelphia College of Pharmacy (PCP) network. Three additional non-core elective rotations may be done within or outside of the current PCP network.

The APPE rotations are offered throughout the year in 5-week long rotations (200 contact hours each). Students will be assigned to 7 of the 8 available rotation periods Students are engaged in orientation and planning their APPE rotations during the entire third professional year. The APPE year begins with a 2 credit APPE preparatory course (PP617) to enhance students' rotation experiences and concludes with a 1 credit campus based course to prepare them for success beyond graduation. APPE rotations begin in late May after all required course work is complete and conclude the following April.

The Advanced Pharmacy Experience rotations are each considered laboratory courses that require a minimum of 200 attendance hours. Forty attendance hours per week for 5 weeks are expected to ensure the successful completion of each specific rotation. Some preceptors may require a weekend or evening commitment. Preceptors will verify student attendance.

Rotation objectives, task lists, and the evaluation parameters provide a structure for learning in core rotation areas. For non-core elective rotations the preceptor will define student tasks and responsibilities unique to the setting. Students receive 35 academic credits and up to 1440 hours of State Board approved internship credit for successful completion of the course series. Students may not receive compensation for their participation in these courses.

APPE Rotation Overview

Acute Patient Care / General Medicine (APC):

Taking place in acute care hospitals where students participate as part of an inter-professional team, this experience emphasizes patient care involvement using a case-management approach, to include: patient data collection, organization, and assessment; development of plans that respond to desired therapeutic objectives; patient monitoring to include physical and laboratory assessment; communication with patients and care givers to acquire patient data, assess treatment outcomes, and drug therapy management. Under the supervision of a preceptor provide education to patients; communicate with and make recommendations to prescribers, and engage other health care providers in delivery of patient care. Students also participate in obtaining medication histories from patients, provide drug information to health professionals, document patient related findings and information and participate in development of a discharge plan. Rotation options include general medicine, critical care, infectious disease, pediatrics, hematology/oncology, organ transplantation.

Ambulatory Care (AMB):

Offered in a variety of ambulatory care settings, including pharmacist managed clinics and physician office practices, this experience emphasizes primary care using a case management approach with an out-patient population, to include: patient data collection, organization, and assessment; development of care plans that correspond to desired therapeutic objectives; patient monitoring, to include physical and laboratory assessment; communication with patients (and care givers) to acquire patient data, assess target outcomes and provide education and communication with prescribers, and other health care providers to optimize outcomes. Patient counseling and training patients to use medication related devices is common in most sites.

Community Pharmacy (COM):

Taking place in community pharmacies with options in independent, chain and food store pharmacies, this experience emphasizes a wide range of student engagement to include: prescription processing and dispensing, supporting self care by advising patients on OTC medications and devices; patient communication and education; supporting pharmacist provision of patient counseling, immunizations and medication therapy management (MTM), communication with prescribers and other health care providers; and pertinent management activities. Building upon previous IPPE experiences, assignments will be made to facilities that can provide the greatest degree of new and unique exposure for a particular student.

Hospital Pharmacy Practice (HPP):

Taking place in hospital-based pharmacy services, this experience emphasizes student engagement in administrative and pharmacy services that support pharmacist involvement in the medication use process to meet needs of patients and other health care providers. Areas of focus are likely to include order processing resulting in dispensing of medications including sterile product preparation and use of technology/information systems to support medication use, projects that may include DUE, formulary monograph or policy development and adverse drug event evaluations, plus supporting pharmacist managed drug protocols and patient care activities such as medication reconciliation, discharge planning, provision of drug information, medication safety initiatives and other quality improvement activities. Activities enable students to understand accrediting organizations, government initiatives (CMS, HRSA, state boards) and payment practices that influence pharmacist services.

Patient Care Elective (PCE):

These non-core experiences provide for individualization and respond to unique student interests. Based on student preference and rotation availability, students will complete a rotation providing patient care services, such as: an additional Acute Patient Care, Community Pharmacy, Ambulatory Care, or HPP rotation. Unique options include Anti-coagulation Services, Cancer Infusion Services, Compounding Pharmacies, Drug Information Services in a hospital, Emergency Medicine, HIV Services, Home IV Infusion, Hospice/Palliative Care, Long-term Care, Mail Order Pharmacies, Military and USPHS prison practices, Nutrition Support Services, Pain Management, Poison Control Centers, Psychiatric pharmacy services, Rehabilitation center practices, and Transplant Services.

Unrestricted Elective (URE):

Note: Two are required and can be patient care (see above) or in pharmacist practices that do not engage directly in patient care

These non-core experiences are less restricted, to provide for an even greater degree of individualization. In addition to repeating any core-type rotation or another PCE option, students can be assigned to a variety of non-patient care environments to include: Pharmacy Organizations; the FDA (Rockville, MD); private Law Practice, Managed Care/Pharmacy Benefit Management companies; Medical Communication companies; Nuclear Pharmacy Providers, Pharmaceutical Industry options, such as medical information, drug safety, clinical research, marketing, or regulatory affairs; Pharmacy Administration in a hospital or chain pharmacy; Investigational Drug Research Services in hospitals, or on-campus research or teaching rotations with PCP faculty.

NOTE: Students may request assignment to Out-of-Network sites for any PCE or URE Rotation. The purpose of this option is to enable students to pursue experience in a unique practice type or geographic areas not provided by the PCP network. Out-of-Network proposals should focus on placements that would build upon (not duplicate) prior intern experiences.

APPE Portfolio

Currently we have implemented an electronic portfolio enabling students to maintain and share their accomplishments and achievements from each individual rotation with future preceptors and even prospective employers. Our goal is that over time we expect the portfolio to be a compendium of accomplishments and competency assessment for all four years of the professional curriculum.
